Please be aware of what your membership provides:

  • If you are a founding member, you will lose your price lock.
  • Founding Memberships (Mon-Thurs Weekday Membership) are not eligible for freezes.
  • Unsure of your membership type, please contact our office for assistance.
  • Special member-only discounts throughout the year, allowing session purchases and extra credits at a steep discounts.
  • Same day guaranteed an appointment when you call before 10 AM.

Membership Cancellation Form

Cancellation Notice: All memberships require a 30 day notice and my membership will be active for the next 30 days which will result in one more additional charge. Any credits on your account remain valid for 180 days from the end of the membership. You will need to submit a separate request for each membership type you are part of.
